Parent & Athlete Checklist

Due By: End of Week 1

To ensure your athlete remains eligible to participate in A.V. Peak Performance Lab, please complete the following items by the end of the week:

Program Requirements

Attend Mandatory Parent Meeting

Join the A.V. Peak Performance Lab Band App

(Required for all program communication.)

Submit Weekly Nutrition Log

(Due every week throughout the program.)

Athlete Clearance & Participation Acknowledgment

I certify that my athlete is physically capable of participating in track and conditioning activities and that all health and participation information provided is true and accurate. I understand that failure to disclose relevant medical information may increase the risk of injury or illness.

Medical Information & Allergy Disclosure

I have fully disclosed all known medical conditions, medications, allergies, dietary restrictions, injuries, and other health concerns that may affect my athlete's participation. I understand it is my responsibility to notify the program of any changes to my athlete's medical status.

Emergency Medical Authorization

In the event of an injury, illness, or medical emergency, and if I cannot be reached in a timely manner, I authorize A.V. Peak Performance Lab staff, coaches, volunteers, or designated representatives to seek emergency medical treatment for my athlete. I understand that all medical expenses incurred are my responsibility.

Photo, Video & Media Release

I grant permission for my athlete's image, likeness, voice, photographs, video recordings, and other media content to be used by A.V. Peak Performance Lab, its affiliates, sponsors, and promotional partners for educational, marketing, promotional, fundraising, website, social media, and other lawful purposes without compensation.

Transportation Permission & Liability Acknowledgment

I authorize my athlete to be transported by approved coaches, volunteers, staff members, or designated team representatives for program-related activities. I understand that reasonable safety precautions will be taken; however, transportation involves inherent risks, and I agree to release and hold harmless A.V. Peak Performance Lab, its staff, volunteers, and representatives from liability except in cases of gross negligence or willful misconduct.

Athlete & Parent Agreement

I have read, understand, and agree to comply with all program policies, attendance expectations, communication requirements, behavioral standards, safety procedures, transportation guidelines, and codes of conduct. I understand that failure to comply may result in disciplinary action, suspension, or removal from the program without refund or scholarship reinstatement.

Please submit a copy of your athlete's birth certificate

Submit Recent Sports Physical (within the last 12 months)

Parent/Guardian Certification & Acknowledgment

By signing below, I certify that I am the parent or legal guardian of the athlete listed on this registration. I have read, understood, and completed all required sections of this packet. I acknowledge that participation in athletic activities involves inherent risks, and I voluntarily allow my athlete to participate in A.V. Peak Performance Lab programs and activities. I understand that it is my responsibility to provide accurate information and to notify program staff of any changes to my athlete's medical condition, emergency contacts, or other relevant information.

I understand that my athlete's participation is contingent upon compliance with program requirements, policies, and safety guidelines.
